This agreement is made between Elisabeth Krijtenburg, working as The Taper Coach ( the Coach) and you, the Client.

Confidentiality



As a Client I understand that everything that we discuss during my support consultations will be held as confidential unless I state otherwise, in writing, except as required by law and except in cases where there is a risk of harm to myself or others or if a crime is committed. 


I understand that due to the cognitive impairment that can be experienced during withdrawal, sessions may be recorded, and I understand that all files of recordings will be accessible only by the Coach and will be fully encrypted and password protected which makes them completely secure. I agree that the recordings will be stored for one year and then will be deleted.

Consultations

I understand that consultations will be in the form of telephone sessions, Skype sessions or voice messages. I understand that if I am more than 10 minutes late for a telephone session, the Coach will assume the session is cancelled and I will forfeit the session fee.

I understand that rescheduling a session can be done with appropriate notice.  I will allow for at least 24 hours notice to appointment changes, and if not, the fee paid will be lost.

I understand that the early termination of a call or failure to answer the phone for an appointment will incur the full charge for the session. 

I understand that by asking for the option of support by messages that takes the Coach more then 15 minutes, I will receive an invoice for those message after having received them. I understand it is my responsibility to pay the invoice for this support within 14 days. 



Support

I understand that the Coach is not qualified to advise or comment on any medical treatment I may be receiving or to recommend and/or supervise tapers off medication.

I understand that this type of coaching support does not involve the diagnosis or treatment of mental or physical disorders.

I understand that a consultation is not a substitute for mental or physical health care and is offered to provide information,encouragement and emotional support only.

Next of Kin

I agree to give the contact details and name of next of kin at the first consultation.

Liability

This agreement shall be construed and governed in all respects in accordance with the Law of The Netherlands and any dispute or differences in relation to this agreement shall be subject to the exclusive jurisdiction of the Dutch Courts. 

Booking your appointment 1) confirms that you, the Client, accept and understand the above Terms and Conditions of this agreement in full and 2) suffices as an electronic signature.
